Do most flights have AED on board? I assume so as they seem to be everywhere.

Yes for airliners. This has been a FAA rule since the early 2000's.  Not every cause of a heart attack is treatable by an AED.  V tach or V fib, two common causes of sudden cardiac arrest, are treatable by shock.

Yes but it's not necessary to switch transponder codes when you're under ATC services.  Declaring the medical emergency is sufficient.
A medical emergency not squawking an assigned ATC code would squawk the same as an emergency. 
7500: Hijack
7600: lost com
7700: inflight emergency

7600 is the most common.  Little guy trying to come into a tower for example, radio goes out, they squawk 7600.

Doesn't matter.  I don't divert for dead crew; I've got places to be.   >:D =D
