Speculation on How This Could Happen

Cop gets hugged, gun goes off, and a lady dies.
The article makes is sound like she hugged the cop from behind, gun goes off and she dies with a bullet in the heart.

I'm honestly puzzled by this story.  Lots of information is missing which would explain the accident. 

The reason I question the events // story is because it is written around the usual "gun goes off and . . . . ."
