Here is the previously thought long-lost video of the actual house votes on the Hughes Amendment and the FOPA back on April 10th, 1986.

As best I could follow the video it went something like this....

Lots of attempted obfuscation by Hughes to keep his amendment from being reading aloud.
Voice vote on Hughes amendment, which Rangel declares passed (listen for yourself).
Demanded recorded vote on Hughes amendment, recorded vote is 124 yay, 297 nay, 14 abstain.
Another voice vote on Hughest amendment, which Rangel again declares passed.
Rangel ignores another demand for a recorded vote.
Voting is conducted on the Volkener substitute and then on the FOPA which both pass.
