How would you set up an ISP in an "unfree" country

In many non-western countries the government controls what its citizens can see and do on the internet by controlling the ISP's. If you wanted to run an ISP let repressed users view whatever they wanted how would you do it, and just as important, how would you do it profitably?

I thought about 2-way satellite, however, the user requires a 2 ft satellite dish and very unique equipment, meaning the government could stop internet access by controlling satellite dish access.

Any thoughts?

...they have been beaming forbidden transmissions into "unfree" countries for years.
I'm sure they've also begun to at least consider the benefits of opening the web to populations of those countries enslaved by oppresive governments.
