That's happened before.
Often enough that there's a rule covering pole breaking.

I wonder if anyone ever got skewered by the broken piece. Seems like a deal like that would be covered up, though.

I couldn't find anything from a broken pole (I really didn't look very hard) but I did find a couple close calls from an unbroken pole doing a vault.

From the looks of things other than being the target during javelin throws pole vaulting looks like it is right up there with dangerous tack and field disciplines.

I vowed I would not make any more comments on some of the utterly stupid foolish dumb irrational events except for the biathlon and the fantastically beautiful athleticism of the ice skating events --definite exceptions.
So I won't mention how utterly stupid foolish dumb and irrational some of the other events are. Nope.

Yeah, Sandi got a Mulligan, but she injured her hip in the fall in that first attempt, and that knocked her out of the competition.
