Offduty Cop's Gun "Fires Twice" in Shopping Mall Bathroom

So obviously there's a lack of info and possibly some shoddy reporting (i.e., the gun seeming to "fire itself"). "The gun fired twice with both bullets hitting the ceiling" indicates to me that the cop was likely fighting for his jacket, or at least trying to get the gun out of it when the shots were fired. My first thought is a striker fired pistol without external safety, but if it was in a jacket pocket (unknown) it seems to me the slide would have likely caught after shot one. Yet a revolver indicates that a finger was deliberately on the trigger, at least for the second shot. I'm making an assumption that the crook did not have control (or full control) of the gun.

I'm trying to imagine how that would happen legitamitely.  Can only imagine there three options off the top of my head.

1.  The coat hook got hung in the trigger.
2.  He tried to reach in and get the gun out of the jacket and had his finger on the trigger.
3.  He fired knowingly to alert someone to the stuggle. 

Is stealing jackets from occupied stalls a new thing?  I have never heard of that.  I would think it would be easier to sneak under the stall wall from the next stall and lift the wallet.
