The NYT has been doing a series on patient safety in American military medicine.  Here is the latest article on the lack of transparency and disclosure on errors in patient care.

They keep making this eternal vigilance thing harder and harder.  Protecting the 2nd amendment is like playing PACMAN - there's no pause button so you can go to the bathroom.
