Video would tell the tale.  If it was just a well time photo of a normal conversation that just looked like what Zero Hedge claims, or really was a quick neuro exam.

Noses look different.  One on the left looks straighter.

Forehead slopes appear a little different, but that could just be the photo angle and lighting.  Change in slope appears much closer to the hairline on the right.

Haircut appears slightly different around the ears.

Face overall looks more slender on the right.

Again, could just be differences in lighting and camera angle.  But I'd give it around 1/3 odds of being different people.

That can change easily in a fairly short period of time due to hydration levels, fatigue, etc.  A quick nap can firm things up, or remove tension.
