Another 2nd stage issue this time with Crew 9's launch. Didn't effect the actual launch itself but there was an issue with the deorbit burn and the 2nd stage came down a bit off target.
SpX is pausing launches until they figure out what happened.

One of the SRBs had obvious issues on the way up and one point it appears the nozzle blew off and you can see the BE-4 gimbaling to compensate. It appears it made it to it's target orbit despite the problem.

You can see burn through on the SRB starting at the 6 second mark. Nozzle says bye bye at ~26
They got extremely lucky here in that the burn through wasn't aimed at the ship* and that the system was able to compensate. You can see the rocket doing a little dance,
