Does anyone see them? I AdBlock the hell out of every site. I haven't seen banner ads in years.

Right. That site is running a version of vBulletin, same software that THR runs. I'm setting up a space for a company that does healthcare education, and they want to have sponsor stuff on the the pages... Including the forum pages...

You should be able to place an ad by changing the php code. I have no clue how to do that but give me enough coffee I could probably figure it out.

How complex do you want to get with the ads?

Rotating Banners?
Subforum specific advertisement?
Clickthrough tracking?
Content-specific advertisement?
End-user administration?
Hit based display limitations? (I.e., ability to sell 10,000 views or 100 clicks, but only display the ad for that many hits)

As Charby mentions, you can hack it into most existing forum software, even if it doesn't initially support it.  Since most forums use a framework, a decent coder could probably set up the most basic banner system for between .25 to 1 hour worth of work, depending on the forum code.

A good deal of prefab PHP software has a great deal of a support and development community. Odds are you can find mods that already do what you want and you simply upload or C&P some code. For example, if you were running the same forums software that APS uses (simple machines), you can use this - http://custom.simplemachines.org/mods/index.php?mod=255
