Part of me is saying "Streetfighter."

 I've been out of the cinema in crowd for six years, with young children. 

I did sneak out for Grindhouse: Planet Terror-Deathproof and had my sensibilities suitably offended.
7-11 was a part time job.

for action violence the latest is Punisher Warzone.

but still, if you want to watch something truely violent and has some intellegance, Clockwork Orange can still shock folks in this day and age.

Goriest movie I've ever seen. Plus plenty of swearing.

I forgot to mention, there's plenty of bayonetting and shooting too.

i never have been one to equate violence with gory. too me stuff like richard III is more violent simple because of the reason behind the violence. whereas a action flick with lots of physical action violence doesn't really raise an eyebrow.

War movies, even the most violent ones don't effect me as much as movies which just have gratuitous gore and violence. Saving Private Ryan has (literally) a lot of guts and blood, but I have more trouble watching the Saw movies, or in fact any with detailed torture scenes.
